Requests

On this page you can request sermon tapes, add to the prayer list, be added to our e-mail list, or to request information. Below is a brief description of each field that can be filled in.

This field can be used as an subject or topic area.

ADDITIONAL COMMENTS

Please feel free to use this to ask a question, expand on a question you have or to give us feedback and comments.

CATEGORY

Use the drop down menu to choose which catagory your information pertains to.

RELATIONSHIP

This drop down menu is intended to be used when you have a prayer request, to let us know how the person or family is related to you.

LOCATION

If your prayer request involves someone in the hospital, then this drop down menu is an easy way to select the appropriate hospital.

ROOM

If someone is in the hopital and the room number is known, it would be helpful if you provide it.

POST TO WEB SITE

Most people will tend not to post the request, but if you wish to choose the yes option then what you have typed will be e-mailed to the church office, reviewed and then posted on the website for anyone to read.

Choosing no the information will not be viewable, but will be forwarded to the church office.

NAME AND E-MAIL ADDRESS

This is a required field to submit a request. We need this in the event that we need to contact you and this information will only be shared with the appropriate personel in the church.
